||Physician Hospitalist (NMMC Hosp Svc)
||The Physician Hospitalist, in collaboration and consultation with other physicians, APPs, staff RN's and other health care professionals, provides holistic, compassionate care to inpatients and their families. The Hospitalist will manage the care of patients hospitalized with complex acute health problems, such as trauma, coronary artery disease, cancer, diabetes and heart failure. The Hospitalist will be a member of the hospital interdisciplinary team and facilitate continuum of care for patients from admission through the discharge process, including facilitating the patient's transfer of care back to the primary provider and consult and make referrals to physicians and other members of the health care team as appropriate. The Hospitalist will also help organize work load and facilitate provider groups to streamline the care of patients on the service.
